Job Purpose
Serve an integral role in helping IKOR clients achieve the highest level of wellbeing possible by assessing and advocating on behalf of client’s physical and mental needs, and collaborating with physicians and other medical professionals.
Job Type
Hours are part-time and schedule is flexible and generally between the hours of 9-5, Monday through Friday. However, must respond to client emergencies at any time; these occurrences are infrequent.
Duties and responsibilities
- Enhance the quality of life for the elderly and disabled through life care management
services that address the medical, financial, psycho-social and environmental needs of
our clients while respecting and advancing clients' personhood. - Job entails assessment, care coordination, care oversight, interaction with routine and
emergency care providers, community resources and families. This is not home health
care. - Provide an onsite medical, environmental and psycho-social assessment of the assigned
client using the IKOR Assessment Forms and Procedures. - Document Care Plan recommendations and work with other IKOR employees and family
members to execute plan. - If requested by client, attend doctor appointments, therapy appointments, facility team
meetings, and/or family conferences as dictated by the needs of the case and the
parameters set forth in the IKOR Agreement of Service. Attend hospital, rehab and SNF
discharge planning meetings with or on behalf of clients. - Interface with all other IKOR team members to ensure smooth coverage of client needs
while avoiding redundant efforts across the team. - Maintain detailed, timely, accurate and dispassionate case notes in client case note
system, and carefully log client-related expenditures to ensure proper reimbursement to
IKOR. - Attend regular meetings of the entire IKOR staff for in-service, training and/or staff
development. - When necessary, provide court testimony or deposition addressing efforts and
observations.
Qualifications
Valid RN license; valid driver’s license; college degree or equivalent experience; willingness to take Montreal Cognitive Assessment (MoCA) training and ability to pass MoCA certification; clear understanding of HIPAA and PHI privacy laws; Microsoft Office Suite experience a plus.
